Attachments include a "CS Survey" which is sent by IES to participants in project or workshop experiences six weeks after the event. The CS Survey is sent only to open enrollment continuing education attendees. Also attached is a "CQS Survey" which is sent by IES to participants in in-plant projects and workshopsÉ.also approximately six weeks after the event. Approximately one year after the event, an independent agency also sends the CQS Survey to all in-plant participants. It is the results of *this* survey that IES uses to report economic impact.

Survey for Workshop Attendees

Attachments include two examples of program surveys that are used to determine the quality of the workshops and seminars. The evaluation forms are filled out at the end of the event and must be returned in exchange for the certificate of completion. Attachments also include two examples of end of year survey forms that are sent to workshop and seminar attendees to evaluate the economic impact of these activities. The results from the evaluation forms are used to improve the program content to better server the customer. The results from the surveys are used to determine economic impact and to decide how the activity might be modified to have a greater impact in the future.
